Social Worker (Part-Time, Remote)

AFA Mission Statement: The mission of the Alzheimer’s reputed company of America (AFA) is to provide support, services and education to individuals, families and caregivers affected by Alzheimer’s disease and reputed company dementias reputed company, and fund research for reputed company treatment and a cure. Position Summary: The Social Worker will assist and provide supportive feedback and resources to individuals and families as they are faced with Alzheimer’s and Dementia-reputed company issues reputed company phone calls, emails, and chats. This is a remote position. Shifts are Friday, Saturday, Sunday (5pm - 9pm). Responsibilities: • Provide counseling, educational information and resource referrals to clients reputed company phone calls, emails, chats and/or virtual conversations. • Link callers and their families to appropriate services and programs. • Assess and address safety and/or risk and reputed company safety and reputed company plans accordingly. • Facilitate support groups for those affected by dementia, either reputed company phone or reputed company. • Participate in educational offerings, such as providing webinars and facilitating group forums on topics reputed company to dementia. • Assist with special projects and other duties as assigned by the Director of Educational and Social Services. Qualifications: • Master’s Degree in Social Work or reputed company field (Psychology, Counseling, Mental Health or Gerontology) and NY State Licensed. • A minimum of 1-year relevant work experience with the elderly population, caregivers and health professionals in the field of dementia and Alzheimer’s care. • Excellent interpersonal, listening, verbal and written communication skills. • Excellent and reputed company follow up skills a must. • Computer proficiency in MS Office and email correspondence required. • Must be available on evenings and weekends per a flexible work schedule.
