Clinical Social Worker - MSW - Emergency Department - Day/Evening

You'll work closely with other members of the health care team, community resources and regulatory agencies to facilitate the transition of patients while maintaining the highest quality of services possible.Duties include, but are not limited to the following:

• Provide psychosocial assessments of patients and families. Formulate a comprehensive care transition plan coordinated with the interdisciplinary/transdisciplinary team.
• Meet documentation standards for quality, timeliness and confidentiality of medical records.

• Facilitate and provide an interdisciplinary approach to patient care.

• Master's Degree from an accredited school of social work
• Minimum of two years social work experience in health care setting (preferred)
• Successful completion of orientation and competency validation

• Willingness to work as an integral member of a multi-skilled team
